Registering a Birth/ Still Birth
place and date of the birth- name, surname and sex of the baby
- parents’ names, surnames and address
- places and dates of parents’ birth
- date of parents’ marriage or civil partnership
- parents’ jobs
- mother’s maiden surname
Information we ask for still birth
As well as the medical certificate issued by the midwife or doctor, we will also ask you to give us:

the date and place of the stillbirth

any names you wish to give the baby
-
the mother’s full name, maiden name (if applicable), date and place of birth, occupation and usual address
-
the father’s full name, date and place of birth and occupation.
If information about the father is not recorded, it may be possible for the stillbirth to be re-registered to include his details at a later date.
